Please read carefully through the following paragraphs.
I hereby authorise Andrea Scott of Brows by Andrea to perform permanent make up upon me. If any unforeseen condition arises in the course of this procedure, I further request and authorise her to do whatever she deems advisable and necessary in the circumstances.
I accept all responsibility for determining the colour, shape and position of the permanent cosmetic procedure.
I understand that an allergy test does not guarantee that I will not have an allergic reaction to the pigment. A patch test must be performed before any permanent make up procedure is performed.
I fully understand and accept that pigments are used during the procedure and that the cosmetic enhancement will fade in time. Even though the colour has faded, the pigment will stay in the skin indefinitely.
I have been informed that the highest standards of hygiene are met, and that sterile disposable needles and PPE are used for each individual client, procedure and visit.
Upon completion of the procedure there may be redness and swelling of the skin which will subside in a few days. In some cases, bruising can occur.
I have been advised that the true colour will be seen 1 month after a procedure, and that the pigment may vary in colour according to skin tones, skin type, age and skin conditions. I understand that some skins accept pigment more readily than others and no guarantee to an exact effect or colour can be given.
To my knowledge I do not have any physical, mental, medical impairment or disability that might affect my well-being as a direct or indirect result of my decision to have the procedure done at this time.
I am at least 18 years old. I am not under the influence of drugs or alcohol. I am not pregnant or nursing. I have not had Botox or fillers in the last 3 weeks.
I agree to follow all pre-care and aftercare instructions as provided and explained to me by Brows by Andrea.
I accept any and all responsibility myself for any consequences that might stem from my decision to have any permanent cosmetics procedure performed by Brows by Andrea.
I understand and accept that each procedure is a process requiring multiple applications of pigment to achieve desirable results and that 100% success cannot be guaranteed and expected results can vary due to the nature of individual skin types and external factors. I understand that this is why I need to return for a top up.
I accept that a top up will be performed 6 - 10 weeks after the initial procedure. Any work completed after 10 weeks will be charged an additional fee.
I have been given the full opportunity to ask any and all questions which I might have about obtaining permanent make up. I also acknowledge that all my questions have been answered to my full and total satisfaction.

I have completed a patch test
All procedures require 2 appointments and a colour boost every 12-24 months to keep the colour fresh
Frequent tanning and sun exposure will fade the pigment quicker.
Alopecia, Thyroid and clients with low iron levels have had explained that pigment retention may not be as good and may require more frequent top ups.
I understand that skin altering procedures, such as plastic surgery, implants and/or injections might alter or degrade my permanent make up. Similarly such changes are not the fault of Brows by Andrea. I further understand that such changes in my appearance may not be correctable through a permanent make up procedure.
When you leave the clinic, hair strokes look intact. How your body heals is out of the control of Brows by Andrea. Even when following the after-care, fading, blurring or poor retention can happen depending on your skin and lifestyle.
In the event of a CAT or MRI scan, please inform your physician of your iron oxide permanent cosmetics as some pulling or burning sensation may occur during the scan procedure.
I understand that if I do not abide by the strict after-care, I can ruin my results. The after-care is crucial for optimum pigment retention.
Permanent make up is an art, not a science. Clients results will vary. We have no control of your body’s healing process.
Brows by Andrea has a no refund policy for any work performed.
If Brows by Andrea believes policies or procedures are not being followed correctly or that information on the medical consent form is not accurate and truthful then further treatment will be refused.
I have read and fully understand the contents of each paragraph above. I acknowledge that this is a legally binding contract and that I have received no guarantees with respect to any permanent makeup procedure that I have done by Brows by Andrea.
